MHN ACO, LLC evolved from Medical Home Network (MHN) and is a formal provider collaborative established in 2009 by the Comer Family Foundation.

The goal of MHN is to develop and implement provider-based solutions that improve the healthcare of Medicaid recipients in Chicago’s south and southwest neighborhoods.
